If you do not see your plan listed, you can still see a Metamorphosis therapist by using your out-of-network benefits. We will provide a superbill if you wish to use out-of-network insurance benefits. If you’re planning to use out-of-network benefits, Metamorphosis partners with Mentaya to make getting reimbursed simple and painless!

We are not in-network with Medicare or Medicaid/Medical Assistance Plans., including any Medicaid/Medicare/Medical Assistance plans managed by the above insurers. Medicaid/Medicare plans do not allow out-of-network billing, but you may pay out of pocket if you wish.

How Does Out-of-Network Billing Work?

Should you wish to see a therapist at Metamorphosis who is not in network with your insurance, you can use your insurance’s out-of-network benefits. We recommend calling your insurance company or using their online portal to confirm specific costs.

Should you choose to use out-of-network benefits, you will pay Metamorphosis out of pocket for your therapist’s full rate at the time of service. Metamorphosis will provide a superbill to you with information necessary to submit a claim. You will reach out to your insurance independently (or through Mentaya) to submit the claim. Should your insurance approve your claim, they will reimburse you directly.

Note that Metamorphosis cannot guarantee that any insurance company will reimburse any claim.

Accepted Forms of Payment

Metamorphosis Counseling accepts all major credit cards and HSA/FSA debit cards. Metamorphosis Counseling does not accept cash or any types of checks. Payment is due at the time of service and will be processed via the client portal during your session. A credit card form will be sent to you with your intake documents to fill out prior to your first session.

You are responsible for payment of your session if you lose coverage, or if your insurance does not pay for the session’s claim. A credit card must be kept on file to begin services.

Why Consider Paying Privately or “Out of Pocket”?

In order to have your therapy services paid for or reimbursed to you under insurance:

  • a mental health diagnosis must be made

  • your insurance has the right to ask for copies of your records (and once they are provided, your therapist cannot control what your insurance does with your information)

  • your insurance has the ability to limit the frequency and length of your sessions, the types of therapy you receive, and where your session is held

  • your therapy must be deemed “medically necessary”, which can limit what you are able to work on in therapy

As a result, some clients choose to pay out of pocket for their services in order to have more privacy and control over their therapy services. If you wish to opt out of using your insurance, please notify us - all you need to do is complete a form. Metamorphosis Counseling respects your choice, and is happy to work with you to come to a decision if need be.

Important information about your rights if uninsured, using out-of-network benefits, or paying out-of-pocket:

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using their insurance an estimate of the bill for medical items and services.

You have the right to receive a “Good Faith Estimate” for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

Make sure your healthcare provider gives you a “Good Faith Estimate” in writing at least one (1) business day before your medical service or item. You can also ask your healthcare provider, and any other provider you choose, for a “Good Faith Estimate” before you schedule an item or service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

Make sure to save a copy or picture of your Good Faith Estimate.
